The mission of SWCHC is to assure quality health services to those lacking adequate resources to access necessary care.
SWCHC serves low income uninsured individuals.
Each year SWCHC provides approximately 2200 visits to 1100 individuals of whom:
100% are uninsured
84% are below 100% Federal Poverty Level (FPL), the equivalent of $24,000 a year for a family of four; 92% are below 138% FPL.
71% are from racial or ethnic minorities, with 55% identifying at Latino/Hispanic.
47% speak a primary language other than English; 43% require an interpreter during visits.
74% are parents, caring for at least one dependent.
41% of all visits were dedicated to one of the following chronic condition: diabetes, hypertension, depression and/or anxiety
Patients pay per visits based on a sliding scale. On average SWCHC collects $7.50 per visit. Each visit includes the following, as needed, at no cost to the patient:
Labs
Imaging
Eye Exams and Glasses
Medication
Insulin and Diabetes Supplies
Referrals to specialty care
Our patients have no alternative for healthcare outside of an emergency room.
